In order to participate in Camp, please follow these steps:

1. Please Take a picture of the Front & Back of your insurance cards.

2. Fill out this medical form, and take a picture of it filled out.

3. Please enter in your Registration ID number below and your email.
And upload the front & back of insurance card, along with your filled out medical form.
